On this page we have posted the various forms that you may want to utilize.  Each form is an Adobe PDF file that you can download and print from your computer.  Click on the form name to access the form.  Beside each file name is a brief description of what the form is used for.

Special Instruction Form - Use this form for noting any special instructions you might have;  i.e. spots, creases or no creases in slacks, buttons that need attention.

 

 

    

Alert Statement - A copy of this signed form must accompany any household items, leather & suede garments, and any gowns (including wedding gowns).

 

 

 

Credit Card Form - This is the form that we use to gather any credit card information that you would like us to link to your account.  This form can be used for customers who currently have no credit card information linked to their account, as well as for customers who simply need to change or update the information that we currently have.  This form can be sent to us via mail, fax, or in a sealed envelope with your next order.

Repair & Alteration Form - Use this form to accompany any alterations that you send to ensure accurate repairs and/or alterations.

 

 

Storage Request Form - Send in this form with any of the garments that you would like us to store.  This is a FREE service that we offer to all of our customers to allow you to have more room in your closets.  We clean the garments when they come in to our facility and then store them in a climate controled room until you request them back.
